Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. Remaining balances not paid by insurance will be due within two weeks of statement date.

Under special circumstances we do retain your credit card number to use for future charges and/or refunds. This information is securely held until your insurance provider has paid their portion of your bill and notified us of the amount that is your responsibility.  At that time any balance, will be charged / returned to your credit card, and a copy will be mailed to you.  This in no way compromises your ability to dispute a charge or question your insurance company’s determination of payment.
